PROVOLT, Ore. (Mar. 17, 2026) — A Grants Pass man was killed Sunday night in a single-vehicle crash along Highway 238 in Josephine County, according to the Oregon State Police.
Troopers responded to the crash at about 9:21 p.m. on March 15 near milepost 12. A preliminary investigation found that a westbound Ford F-150, driven by 33-year-old Jacob David Bailey of Grants Pass, failed to negotiate a curve, left the roadway and rolled multiple times.
Bailey was declared dead at the scene.
The highway was impacted for approximately four hours as crews conducted an on-scene investigation. Applegate Fire District assisted at the scene.
The crash remains under investigation.
The Oregon State Police Collision Reconstruction Unit, a specialized team tasked with investigating fatal and serious injury crashes, responded to assist. The unit uses advanced techniques, including aerial drone mapping and forensic analysis, to document and reconstruct complex collision scenes across the state.
